Home sleep study versus laboratory sleep study

Home sleep apnea test

A home sleep apnea test in NYC generally measures airflow, breathing effort, oxygen level, pulse and related signals. It is intended primarily for selected uncomplicated adults with signs suggesting moderate-to-severe obstructive sleep apnea.

Advantages: convenience, sleeping in one’s own bed, lower disruption and potentially faster access.

Attended polysomnography

An in-laboratory study records additional signals such as brain waves, eye movement, muscle activity, heart rhythm and limb movement. It may be preferred when central sleep apnea, hypoventilation, narcolepsy, parasomnia, seizure, significant cardiopulmonary disease or another complex disorder is suspected.

How obstructive sleep apnea is treated

CPAP, APAP and BiPAP

Positive airway pressure keeps the airway open during sleep. W8MD may prescribe or coordinate PAP therapy and help address mask fit, pressure comfort, dryness, leak and adherence problems.

Oral appliance therapy

A custom mandibular advancement device may be appropriate for selected patients, often with mild-to-moderate OSA or difficulty using PAP. Qualified dental and sleep follow-up is important.

Weight management

Weight reduction often improves OSA severity in patients with overweight or obesity, but it may not eliminate the disorder. PAP should not be stopped solely because weight has decreased; reassessment may be necessary.

Other treatments

Positional therapy, treatment of nasal obstruction, reduced alcohol near bedtime and selected airway surgery or neurostimulation may be considered according to anatomy and severity.

Why obesity and sleep apnea should be treated together

The relationship is bidirectional. Weight gain can increase fat around the tongue, neck and abdomen, worsening airway obstruction. Fragmented sleep and daytime fatigue can make healthy eating, activity and weight maintenance more difficult. Short sleep can also affect appetite, food reward and glucose regulation.

Treating OSA improves breathing and sleep-related health, but PAP alone should not be promoted as a weight-loss treatment. Likewise, weight loss may improve apnea but should not replace prescribed PAP without reassessment. An integrated plan addresses both conditions simultaneously.

Zepbound for obesity-related obstructive sleep apnea

The FDA approved Zepbound (tirzepatide) for moderate-to-severe obstructive sleep apnea in adults with obesity, in combination with reduced-calorie nutrition and increased physical activity. In the approval trials, weight reduction was accompanied by improvement in OSA severity.
